The problem is that people usually forget to check the chamber when they mess around with the gun and think that ejecting the clip is all that needs to be done, It reminds me of mgs5 when you reload it insert and new clip and then slide a single bullet into the chamber manually to ensure max capacity, or when people are playing Half life alyx and assume they find a bug when they realize they have 11 bullets in the pistol when they reload an empty clip with 1 bullet in the chamber.

Four reasonably simple rules for handling firearms:

1. Keep your finger off the trigger until you are ready to fire.
2. Treat every firearm as if it is loaded.
3. Always be sure of your target and its surroundings.
4. Never point the muzzle of your gun at anything you're not willing to destroy.
