    It's the length/width of the snout. Nose size tends to correlate with gender, a fuller, longer snout (or a larger nose in humans) makes the whole face look more masculine.
    The big issue here is that the artist went to heavy on the anatomical correctness of the horse face, to make her look feminine her snout needs to be softened considerably, thinner and shorter, and there needs to be some suggestion of lips.

    Horses actually HAVE lips, the absence of them creates an extremely masculine appearance. Check out the DCAU animation style, the men are never given lips, if you added lips to them they'd look feminine. Meanwhile if you took the lips off of characters like Lois Lane or Catwoman their sensuality would be severely lessened.
